Unite Us Community Partners Presentation

The St. Francois County Community Partnership, in collaboration with Parkland Health Center and Unite Us, invites community partners to attend an informational presentation focused on improving coordinated care and strengthening referral pathways across our region.

Professionals across education, healthcare, nonprofits, social services, and public safety frequently encounter barriers when attempting to connect individuals and families with needed resources. This presentation is designed to address those challenges by introducing Unite Us, a coordinated care platform that connects healthcare and community organizations to streamline referrals, enhance communication, and foster stronger collaboration.

• The benefits of joining the coordinated care network
• How organizations can register and begin using the platform

This session will provide practical insight into how Unite Us can help agencies work together more efficiently to better serve individuals and families throughout the region.
